Caring for People With Dementia

 

Live Well at Home provides a specialist Dementia care service in Gloucestershire, helping people with Dementia, and their main carers, live well at home for as long as possible.

All our care-workers are trained in Dementia care and understand the key elements to helping people, and their families, live at home with Dementia.

 

- Person centred care - building a relationship with both the service user and their families, allowing time to get to know and understand the service user and be flexible in the time given to each visit. 

- Continuity of care - being visited by the same care-workers on a regular basis to build trust and an understanding of behaviour and any anxieties the service user may have.

- Integration between other health and social care teams to provide a holistic approach to care.

- Sensitive to the needs of the service user's main carer and recognition that they are the best source of information for building a picture of the service user's likes, dislikes and personal history.

- Willingness to liaise with the main carer to keep them informed and to listen to any concerns they may have.

- Provision of a 24 hour emergency care service, giving both the main carer and service user peace of mind that help is at hand any time of the night or day.

 

- Provide help in ensuring the main carer has access to respite care, by allowing the service user to remain in their own home with the support needed to ensure they are safe and secure.
